FMCSA publishes seven recurring reasons an MCS-150 submission may be incomplete and not processed. Some of the wording comes from paper-form fields, but the underlying lessons remain useful in Motus: use current instructions, prove identity, keep names and roles consistent, and do not certify contradictory data.

A filing can be accepted and still contain harmful mistakes
Passing required-field checks does not prove the operating profile is accurate. A mistaken mileage year, inflated power-unit count, incorrect driver count, wrong cargo selection, or bad principal-place-of-business address can post to the public record and create downstream questions.
- Reporting one vehicleâs mileage instead of total fleet mileage.
- Entering current partial-year mileage when the requested year is the prior completed year.
- Counting trailers as power units or mixing ownership/lease categories.
- Excluding the owner-operator from the driver count when the owner drives.
- Selecting authorized-for-hire without matching authority.
- Using an accountant, consultant, virtual office, or mail drop as the principal place of business without confirming it qualifies.
- Copying old cargo classifications after the operation changed.
Motus access problems that happen before the filing begins
Many apparent âMCS-150 errorsâ are actually access or identity problems. The user may have the wrong Login.gov email, may not be the company official, may have created the wrong account type, or may not have completed the company-claim process.

Choosing the wrong registration action
A routine biennial update is not the right answer for every problem attached to a USDOT number. The company may need reactivation, New Entrant reapplication, authority reinstatement, a legal-name change with documents, an out-of-business action, or the specialized MCS-150B/C path.
When the public record and the intended action disagree, pause before submitting. A second incorrect filing can make the history harder to understand and may not resolve the original issue.
Duplicate and out-of-sequence submissions
Submitting the same information through multiple channels does not necessarily make processing faster. A later transaction can conflict with an earlier pending action, and the company may not know which version will control.

- Use the transaction reference when contacting FMCSA.
- Respond to the specific document or correction request rather than opening an unrelated action.
- Keep a copy of exactly what was submitted and when.
What if the update is not showing in SAFER?
First determine whether Motus shows the action as submitted, pending, returned, or completed. A completed intake with a private provider is not the same as a completed government transaction. If Motus shows completion but material public fields remain unchanged after normal processing, use the official tracking reference when requesting support.
- Confirm that the transaction was actually submitted, not saved as a draft.
- Check email and Motus for a return notice or supporting-document request.
- Verify that the correct USDOT record was used.
- Compare the fields that were supposed to change with the current public record.
- Contact FMCSA with the transaction reference when the completed result and public record materially disagree.
How to correct an MCS-150 mistake
Use the correction path that matches the error. A simple current-data correction may be handled as a registration change. A legal-name, EIN, entity, authority, or ownership issue may require documents and separate actions. Never solve a mismatch by inventing information that makes the form pass validation.
Final pre-submission quality check
- Correct legal entity and USDOT number.
- Correct action: update, change, reactivation, reapplication, or inactivation.
- Current official filing path and form variant.
- Verified company official and authorized user access.
- Legal name and principal address match supporting records.
- Operation, cargo, authority, and hazardous-materials answers are consistent.
- Power units, drivers, mileage total, and mileage year are supported.
- Required ID and documents are legible and belong to the correct people/entity.
- No unresolved pending transaction for the same action.
- Confirmation and submitted-data copy will be retained after certification.

No. Required-field validation cannot verify every operational fact. The registrant remains responsible for accurate information.
