Common MCS-150 Mistakes, Returns and Filing Delays

Quick answer: The most preventable problems fall into four groups: using the wrong or expired filing path, failing identity or signer checks, entering inconsistent registration data, and starting duplicate transactions before the first action is resolved.

FMCSA’s seven published incomplete-filing problems

FMCSA publishes seven recurring reasons an MCS-150 submission may be incomplete and not processed. Some of the wording comes from paper-form fields, but the underlying lessons remain useful in Motus: use current instructions, prove identity, keep names and roles consistent, and do not certify contradictory data.

Published issueWhy it matters
Expired formFMCSA may reject an outdated version. Use the current official form or current Motus action.
Unacceptable or missing IDThe signer’s identity and company-officer relationship must be verifiable when ID is required.
Legal business name mismatchThe name should match the record unless a documented name change is being requested.
Interstate/authorized-for-hire inconsistencyAuthorized-for-hire selections must be consistent with the authority the company holds or seeks.
Vehicle/cargo classification inconsistencyCertain vehicle and cargo answers must correspond rather than contradict one another.
Missing or incorrect officer signatureThe authorized signer must be identified correctly and match the company-officer information.
Filing-date conflictA later paper submission may conflict with an online update if dates and sequence are wrong.

A filing can be accepted and still contain harmful mistakes

Passing required-field checks does not prove the operating profile is accurate. A mistaken mileage year, inflated power-unit count, incorrect driver count, wrong cargo selection, or bad principal-place-of-business address can post to the public record and create downstream questions.

Motus access problems that happen before the filing begins

Many apparent “MCS-150 errors” are actually access or identity problems. The user may have the wrong Login.gov email, may not be the company official, may have created the wrong account type, or may not have completed the company-claim process.

Avoid credential workaroundsDo not create multiple profiles or share one-time security codes simply to bypass an access problem. Correct the account role and identity path.
SymptomWhat to investigate
Company does not appearWhether the correct company account was claimed and the USDOT number matches
User can log in but cannot fileThe user’s company role and authorized permissions
Identity verification failsID condition, lighting, image quality, personal-data match, and session timeout
Wrong email controls the recordThe company-official email used in the former Portal and current Login.gov profile
Application says inactiveWhether the message refers to a transaction, the USDOT record, or another registration action

Choosing the wrong registration action

A routine biennial update is not the right answer for every problem attached to a USDOT number. The company may need reactivation, New Entrant reapplication, authority reinstatement, a legal-name change with documents, an out-of-business action, or the specialized MCS-150B/C path.

When the public record and the intended action disagree, pause before submitting. A second incorrect filing can make the history harder to understand and may not resolve the original issue.

Duplicate and out-of-sequence submissions

Submitting the same information through multiple channels does not necessarily make processing faster. A later transaction can conflict with an earlier pending action, and the company may not know which version will control.

What if the update is not showing in SAFER?

First determine whether Motus shows the action as submitted, pending, returned, or completed. A completed intake with a private provider is not the same as a completed government transaction. If Motus shows completion but material public fields remain unchanged after normal processing, use the official tracking reference when requesting support.

  1. Confirm that the transaction was actually submitted, not saved as a draft.
  2. Check email and Motus for a return notice or supporting-document request.
  3. Verify that the correct USDOT record was used.
  4. Compare the fields that were supposed to change with the current public record.
  5. Contact FMCSA with the transaction reference when the completed result and public record materially disagree.

How to correct an MCS-150 mistake

Use the correction path that matches the error. A simple current-data correction may be handled as a registration change. A legal-name, EIN, entity, authority, or ownership issue may require documents and separate actions. Never solve a mismatch by inventing information that makes the form pass validation.

Frequently Asked Questions

Why was my MCS-150 returned?

Common reasons include an expired form, missing ID, name mismatch, inconsistent classifications, signer problems, or a filing-date conflict. Read the specific return notice.

Can I submit another filing immediately?

First check whether the original is pending or returned and respond to the actual issue. Duplicate transactions can add confusion.

Why can I log into Motus but not see my company?

The company may not be claimed under the correct user profile, or the user may lack the necessary company role.

What if I entered the wrong mileage?

Determine the correct total and reporting year, then use the appropriate record-change or correction action. Keep support for the corrected figure.

Why is my update not showing in SAFER?

Confirm the government transaction was submitted and completed, check for return notices, and use the tracking reference if the public record remains inconsistent.

Can a consultant sign as the company officer?

The filing must be certified by an authorized person in the role permitted by the current instructions. A consultant’s involvement does not automatically make that person the company officer.

Does an accepted filing prove every answer was correct?

No. Required-field validation cannot verify every operational fact. The registrant remains responsible for accurate information.

How do I avoid using an expired form?

Use the current Motus action or download the current form directly from FMCSA immediately before filing.
